Job Title: Band 5 Community Nurse

Driving Licence and Own Vehicle Required

We are seeking a proactive and dedicated Band 5 Community Nurses to join our team, supporting patients in their homes, care homes, and other community settings across Plymouth and surrounding areas.Your Role and Responsibilities

As a Community Nurse, you will:

  • Deliver a wide range of clinical nursing duties to patients in their own homes, care homes, and other settings

  • Assess, triage, plan, implement, and evaluate care, including sensitive and unpredictable situations, with support from senior nurses or caseload holders

  • Support the coordination and delivery of care, applying clinical judgement in response to rapidly changing patient needs

  • Respond effectively to immediate or crisis situations in partnership with the wider healthcare team

  • Regularly use clinical reasoning and problem-solving skills in challenging environments

  • Develop and maintain nursing competencies, ensuring that the changing health needs of patients are met

  • Accurately document care using both paper and digital record systems, adhering to legal and professional standards

  • Provide clinical supervision to peers, students, and less experienced staff

  • Engage in advanced communication and complex discussions with patients and families, particularly around care planning, escalation plans, and end-of-life decisions (training will be provided as needed)

This is an active role that includes:

  • Frequent travel between patient homes in all weather and seasons

  • Manual handling of equipment and nursing tasks that involve physical exertion, such as kneeling, bending, lifting limbs or water containers

  • Use of moving and handling equipment, such as hoists, often in confined spaces and on variable surfaces

  • Risk assessing and occasionally moving light furniture to carry out procedures safely

  • Carrying and transporting essential clinical equipment, including district nurse bags, syringes, and diagnostic tools

Essential Requirements

  • Registered Nurse (Adult) with a valid NMC registration

  • Valid UK driving licence and access to a reliable personal vehicle

  • Practical training certificates

  • DBS with update service

  • Demonstrated experience or placement within Community Nursing settings

  • Commitment to ongoing personal and professional development

  • Ability to work independently and make autonomous clinical decisions

  • Strong interpersonal and communication skills, particularly for supporting patients with frailty and complex needs
