The post holder will be part of a highly motivated team working autonomously but with close support and supervision from a Senior Clinician, delivering high-standard quality clinical care in community settings.
Responsibilities:
- Assess, diagnose, develop and implement individualized care programmes for patients with a wide variety of clinical needs, including acute or chronic neuropathies, chronic diseases, vascular problems and wound management.
- Plan and manage own caseload, providing specialist care for patients with diabetes and other conditions that put them at high risk of foot ulcers.
- Work across multiple community settings such as community hospitals, clinics, schools, nursing homes and primary care settings, and within patients' homes.
Qualifications:
- Qualifying or returning to practice in summer 2024, or exploring the chance to return to practice.
- Ability to work autonomously while collaborating with a Senior Clinician.
