Provide Community in Chelmsford is seeking a dedicated nurse to join the Primary Health Care Team.
The post holder will plan, allocate and review patient care, manage caseloads and develop care plans to meet needs across Chelmsford ICT and potentially other bases.
The role supports the community matron, leads in assessment, implementation and evaluation of care, and ensures effective communication with social services and other agencies while upholding the NMC Code and PREPP.
