About us:
Halstead Community Ward is a 20 bedded Intermediate Care (IMC) unit, specializing in rehabilitation, at Halstead Community Hospital. It is a nurse led unit, where patients are admitted onto a two-week rehabilitation pathway. D2A (Discharge to Assess) and Step Up patients from community settings are also accepted to the unit. Patients admitted to Halstead Ward are supported to reach their optimal level of independence. Our therapists work with patients to set their own goals enabling them to return home, or into an interim placement depending on their individual needs.
