We are looking for registered nurses to work for us as part of our Integrated Care Teams and will participate in the assessment, implementation and evaluation of care to patients in their own homes and will be committed to the development of the Primary Health Care Team.
Main duties of the job:
- The post holder will work across the planned and unplanned pathways delivering care to housebound patients.
- They will act as a professional and clinical role model demonstrating and upholding the values of the organisation and profession as well as behaviours to facilitate a culture of trust, openness, integrity, kindness, and mutual support.
- They will act as an ambassador for Provide and demonstrate excellent customer care.
