Provide is looking to recruit an Outreach Health Check Worker to deliver the NHS health checks programme throughout Essex. The role will include delivering the NHS Health checks at various community sites in Essex to support GP surgeries. You will need to follow best practice guidelines, communicate results to the patients and offer advice on how patients can improve their lifestyle.
Responsibilities:
- Engage with identified clients to deliver the NHS Health Check.
- Provide agreed health improvement advice and signpost information to patients.
- Make relevant referrals to the Essex Wellbeing service following the health check.
- Drive NHS mobile unit to locations in Essex when required.
- Attend community wellbeing events to deliver the NHS Health check.
- Make NHS Health check bookings using NHS clinical systems.
Qualifications / Experience:
- Relevant qualification in health related discipline and/or experience.
- Driving licence.
- Holds Full driving Licence & has access to a car.
- Understanding of cardiovascular disease and its risk factors.
- Professional UK clinical registration required for the role.
