Summary
Are you an enthusiastic and compassionate Nursing Associate/Practitioner looking for a rewarding role where you can make a real difference? Join our Urgent Community Response Team (UCRT) and help deliver high-quality, patient-centred care in the community, supporting people to remain safely in their own homes and avoiding hospital admissions.
UCRT is a 7-day service providing rapid assessment and treatment for patients experiencing a health crisis in their own homes, including nursing and residential care homes. Working closely with partners across Mid and South Essex—we provide holistic assessments within two hours of referral and deliver timely, coordinated care.
As a Nursing Associate/Practitioner, you will work both independently and as part of a skilled multidisciplinary team, assessing and treating patients in their own homes. Your role will include clinical assessments, monitoring observations, venepuncture, catheter care, supporting patients following falls, and managing acute episodes of illness. You will also carry out basic equipment assessments to support our Allied Health Professional colleagues, with full training provided.
This is an exciting opportunity for a proactive clinician who enjoys autonomous working and is passionate about delivering excellent community-based care.
UCRT operates 7 days a week between 8:00am and 10:00pm. The role includes weekend and bank holiday working on a shared rota.

- Provide support to the members of UCRT which include nurses, allied health professionals and healthcare support workers to ensure that holistic assessments are completed whilst working collaboratively with the team to facilitate the treatment of patients within their own home.
- Work with minimal supervision, adhering to organisational policies and procedures and in line with their scope of practice competency, while continuing to develop in the role through the acquisition of further skills, knowledge and competencies to be determined by the needs of the patients, service and team.
