We are looking for Support Practitioners to join our team to work flexibly across our Enablement and Care services. Please note that the majority of our current requirements are short-notice requests.
Fern Lodge is a small care home providing living accommodation for up to 8 adults with learning disabilities, set in extensive grounds and gardens, where everyone has their own large room with bedroom, living area and ensuite bathroom. There are currently six people living at Fern Lodge with a variety of needs. Your working day will be varied, interesting and rewarding, with opportunities to participate in a variety of training opportunities and develop yourself. Within the same grounds is Cypress Gardens, incorporating 8 individual supported living flats where you will enable individuals to develop their ability to maintain independence and grow in confidence through support and encouragement. There are a variety of shifts and hours available including weekdays, weekends and waking nights to flexibly meet the needs of the people who live here.
Main duties of the job:
- Support individuals with learning disabilities to the highest standard towards meeting agreed outcomes.
- Support individuals to maintain their own home, living a fulfilled and happy life.
- Support individuals with personal care ensuring privacy, dignity and respect are maintained.
- Support individuals to create and maintain a living environment that feels like their own home.
- Support and encourage individuals to make choices and life decisions in line with the Mental Capacity Act requirements.
- Assist individuals to engage in everyday home, social, leisure and work activities.
- Support individuals with their health needs.
- Keep up to date and accurate records, maintaining confidentiality.
- Support individuals with their finances.
- Look for opportunities to use assistive technology to increase the person's independence.
- Ensure that any incidents, particularly those which may relate to safeguarding, are reported.

If we require you to hold UK professional clinical registration to undertake a role we appoint you to, Provide will pay for your registration during the first year of your employment.
