As the Multidisciplinary Team (MDT) Coordinator you will be part of the Integrated Care Team providing non‑clinical support to the Clinical Manager, Community Matrons and District Nursing Teams. You will coordinate a team of professionals from multiple disciplines to deliver proactive, person‑centred care for patients with complex needs and frailty.

Key Responsibilities

  • Manage a caseload of frail or complex patients and create MDTs around each patient.
  • Lead multidisciplinary discussions to set and implement individual care plans.
  • Act as the single point of contact for health and social care professionals to access the Frailty Pathway.
  • Maintain an up‑to‑date knowledge of local services, support networks and community resources.

Qualifications & Experience

  • Relevant health or social care experience with complex care planning.
  • Strong collaboration and facilitation skills across multiple disciplines.
  • Knowledge of local frailty services and community pathways.
