Location: Billet Lane, Work will be carried out in people’s homes across all North Lincolnshire, teams work in geographical areas. The registered contractual base is Billet Lane Scunthorpe.

Overview
The North Lincolnshire Home First Community Rehabilitation service is a registered Rehabilitation and Reablement service that provides time limited intervention and therapies to people within their own homes. We strive to deliver support that puts people at the centre of our services. Our aim is to enable people to remain at home, be confident and safe to live independently.
Responsibilities
- Form part of the expanding transitions team to support smooth and effective transition from hospital, providing protected/dedicated visits for discharge from SDEC and A&E.
- Deliver extensive care and support to service users within the community, helping them reach optimal fitness to participate in the rehabilitation program.
- Provide rehabilitation and reablement support to individuals following accident, incident or illness, following therapy plans and working collaboratively with other health and social care professionals.
- Respond to crisis situations such as falls and carer breakdown.
- Assist with moving and handling, attend to personal care needs, administer medication, and maximize opportunities for social inclusion and emotional wellbeing by signposting to external organisations and professionals.
Working Conditions
25 hour per week contract on a rolling rota basis. Working 3 to 4 shifts per week with shifts between 07:00 and 23:00. Includes weekend work with additional enhancements. Opportunity for overtime.
Qualifications
A previous qualification in care is desirable, though not essential. Full induction and training will be provided.
