The predicted salary is between 30000 - 40000 £ per year.

We serve adults from the ages of 18 upwards including people with physical disabilities, older people, and people with Learning Disabilities. We are part of an integrated community team and as a Social Care Assessor you will be working closely with our health teams to support people to improve their life outcomes.

What we’re looking for:

  • Applicants who will be confident in working with people in sometimes difficult situations, their carers and other professionals.
  • Experience of working in a health or social care environment and a sound understanding of human needs.
  • Ideally have experience of undertaking assessments.
  • Able to manage your work individually as well as work as part of a team.
  • An effective communicator, both verbally and through written formats.
  • Able to prioritise workload, manage conflict effectively, be flexible and enthusiastic.
  • Effective time management skills and able to work under pressure.
  • IT literate and willing to develop your skills.
  • Committed to equality and diversity, effective customer service and achieving positive outcomes for vulnerable people.
