Overview
We have adopted the Discharge to Assess Model which is facilitating complex discharges from the hospital. You will be based at Royal Devon University Hospital, supporting patient discharge from hospital as well as assessing service users post-discharge to formulate long-term care arrangements. You will be working collaboratively with our hospital and community-based colleagues within an integrated system.
The role would provide an excellent opportunity for anyone who is looking to progress their social care career. Experience of working within adult social care settings is advantageous.
This role would suit an individual looking for a fast paced and varied role that will include the completion of strengths-based Care Act Assessments, Mental Capacity Assessments, and ongoing care and support planning. You will also have the opportunity to support and advocate for service users on end of life and Continuing Healthcare pathways.
The nature of this work requires you to be highly organised with strong recording skills. You should be an effective communicator both verbally and through written formats, able to prioritise workload, manage conflict effectively, be flexible and enthusiastic, have good time management skills, and be able to work under complex and challenging situations. You need to be IT literate and willing to develop your skills. You will be committed to equality and diversity, to good customer service, and to achieving positive outcomes for vulnerable people. Applicants must be willing to travel and be able to cover a large geographical area (exceptions may apply under the Equality Act 2010). You will be required to be available for Saturday working (approximately 1 Saturday required every 10 weeks – subject to change and business need). You will receive a day of in lieu for Saturday working and an enhanced rate of pay.
