Our Service The Community Health & Social Care team covers a large area, including rural communities, towns, and villages across North Devon. Our team is closely connected with NHS teams, other social care teams, district councils, housing, and equipment providers. We are an enthusiastic, supportive team comprising social care assessors and both band 5 and 6 occupational therapists. The team occasionally carries out assessments via telephone or virtually, but most work is face-to-face in clients\’ homes. About the role You will join a friendly, supportive, and experienced team with regular peer support, supervision, appraisal, team meetings, and a commitment to learning and development, including access to CPD sessions. You will have the opportunity to travel around the scenic Torridge area of Northern Devon to assess and support clients. Your responsibilities include assessing the needs of adults with various health conditions, including physical and learning disabilities, in the Torridge area. Clinical duties involve enabling individuals to maximize independence and safety, including risk management and reduction. You will undertake moving and handling assessments and reviews, utilizing an optimal handed care focus. Housing assessments and recommendations for minor and major adaptations are also part of your role, working with equipment suppliers and local councils for disabled facilities grants. You will identify equipment and technology solutions to promote independence and safety, consider preventative measures and safeguarding, and support clients in achieving their goals. You will join the locality Occupational Therapy duty rota for timely interventions and support the team manager with triaging the Occupational Therapy waiting list. The role offers autonomy and hybrid working options, with support from the team and opportunities for joint working with NHS rehabilitation, learning disabilities services, and wider social care and health teams. We encourage student placements, and you may be asked to host Occupational Therapy students individually or as part of the team. About you You should manage a caseload and deliver a person-centered service through clinical assessments, aligned with local government policies and legislation.
