This is an exciting opportunity if you are interested in progressing your career in the social care field. You will be working in a community Health and Social care team to offer advice and complete assessments for younger adults and older people, primarily through face-to-face contacts within people’s own homes or within care settings. You will be supporting people to retain their independence, undertaking holistic assessments and support planning, working in partnership with other agencies, making onward referrals, and reviewing services.

Essential details: This role requires- DBS disclosure.- Ability to fulfil all spoken aspects of the role with confidence and fluency in English.
