Social Care Reablement is a short-term intervention service supporting people back to independence, with a goal focused approach. The role of a Social Care Assessor within the team is a varied post to support the service with ongoing assessments. You would also be supporting Care Direct Plus in helping people retain their independence, providing advice and information, undertaking holistic assessments and support planning, working in partnership with the public, professionals and other agencies, making onward referrals, and reviewing services.

We are looking for applicants who will be confident in working with people in sometimes difficult situations, their carers, and other professionals. You will have experience of working in a social care or health environment and have a good understanding of human needs. Ideally, you will also have experience of undertaking assessments. You will be able to manage your work individually as well as work as part of a team.

You should be an effective communicator both verbally and through written formats, able to prioritise workload, manage conflict effectively, be flexible and enthusiastic, have good time management skills, and be able to work under pressure. You need to be IT literate and willing to develop your skills. You will be committed to equality and diversity, to good customer service, and to achieving positive outcomes for vulnerable people.

This position will be co-managed between the Social Care Reablement team and Care Direct Plus and you will be part of a friendly, supportive and experienced team with regular supervision, appraisal, regular team meetings, and access to learning and development opportunities.
