We are seeking an experienced Social Care Assessor to join our supportive and forward‑thinking Adult Social Care team. In this full time position, you will play a key role in delivering high quality, person‑centred social work practice. You will work with adults with a range of complex needs, empowering individuals to live independently and safely while ensuring statutory duties and best practice standards are met.
Key Responsibilities
- Undertake strengths based assessments and develop personalised care and support plans.
- Work collaboratively with health partners, voluntary services, carers, and families.
- Manage a varied and complex caseload with confidence and professionalism.
- Apply legislation including the Care Act 2014, Mental Capacity Act 2005 and safeguarding procedures.
- Make defensible decisions that promote wellbeing and positive outcomes.
- Maintain accurate and timely records in line with organisational and professional standards.
About You
- Confident in working autonomously while being a strong team player.
- Able to build effective relationships with service users, families, and partner agencies.
- Passionate about delivering excellent social work practice and continuous improvement.
- Skilled in assessment, risk management, and have a good understanding of safeguarding adults.
- Fluent in English and able to fulfil all spoken aspects of the role with confidence.
