Location: The post is based within the North Devon Community Team, with flexibility to work across localities depending on service need. Our bases include South Molton, Holsworthy, Bideford, Barnstaple and Ilfracombe.
About the role: As a Community Occupational Therapist, you will use core occupational therapy skills and clinical expertise to support adults with complex needs to maximise their independence and achieve outcomes that matter to them.
Responsibilities:
- Undertake strength-based, person-centred assessments in community settings, both face-to-face and via telephone.
- Lead on complex cases, including safeguarding, care planning and risk management.
- Recommend and provide equipment, environmental adaptations and reablement-focused interventions.
- Work collaboratively within multidisciplinary teams across health, social care, housing and the voluntary sector.
- Support hospital discharge, admission avoidance and community-based rehabilitation.
- Provide clinical supervision and support to Band 5 Occupational Therapists, non-registered staff and students.
- Contribute to service development, training and the promotion of occupational therapy practice.
Qualifications & Requirements:
- A qualified Occupational Therapist with HCPC registration.
- Experience in working with adults in a community health and/or social care setting.
- Confidence managing complex cases, risk and safeguarding responsibilities.
- Skills in assessment, activity analysis, moving and handling, and adaptive solutions.
- Ability to work autonomously while contributing positively to a supportive team environment.
- Committed to continuous professional development and reflective practice.
- Full UK driving licence and access to a car is required (reasonable adjustments will be considered in line with the Equality Act 2010).
- Fluency in English.
- UK Right to Work required; visa sponsorship is not available.
- DBS disclosure required.
