Key Responsibilities & Duties:
- Coordination & Planning: Act as a single point of contact, helping families navigate services and coordinate with education, health, and social care professionals.
- Personalized Support: Work with individual children on their specific needs, including physical care (toileting, hygiene), emotional well-being, social skills, and learning.
- Plan Development: Assist in creating and reviewing Education, Health and Care (EHC) Plans and care/learning plans with meaningful, achievable goals.
- Family & Community Links: Build strong relationships with parents, involve them as partners, and help families expand social networks.
- Inclusion & Development: Support participation in all activities, promote inclusion, and help develop physical, intellectual, emotional, and social skills.
- Daily Tasks: Assist with daily routines, including managing behaviour, supporting transitions, and providing encouragement.
