Key Responsibilities & Duties:

  • Coordination & Planning: Act as a single point of contact, helping families navigate services and coordinate with education, health, and social care professionals.
  • Personalized Support: Work with individual children on their specific needs, including physical care (toileting, hygiene), emotional well-being, social skills, and learning.
  • Plan Development: Assist in creating and reviewing Education, Health and Care (EHC) Plans and care/learning plans with meaningful, achievable goals.
  • Family & Community Links: Build strong relationships with parents, involve them as partners, and help families expand social networks.
  • Inclusion & Development: Support participation in all activities, promote inclusion, and help develop physical, intellectual, emotional, and social skills.
  • Daily Tasks: Assist with daily routines, including managing behaviour, supporting transitions, and providing encouragement.

  • Complex Needs: Working with children and young people with the most complex needs.
  • Coordinated Approach: Ensuring support is timely, personalised, and delivered in a coordinated way across different agencies.
