Staff at Prospect Medical Group are dedicated to providing excellent patient-centred care and working with our diverse community. The role will include long-term condition management, childhood immunisation, cytology, smears, and travel vaccinations. We are looking for a highly capable, enthusiastic, and motivated person. The successful candidate will also be confident, have excellent organisational skills, and be able to self-manage.
Main duties of the job
- To provide clinical nursing service and support to other professionals and to patients of the practice including treatment, screening, chronic disease management and advice within the professional competence of the post holder.
- Support and work effectively within the clinical team.
- All duties will be carried out in accordance with the NMC Code of Professional Conduct and with regard to the various practice protocols.
- The candidate will be part of a nursing team responsible for ensuring that the practice provides an excellent level of service of nursing care relevant to the practice population.

Prospect Medical Group is a long established, friendly and forward-thinking GP practice in the west end of Newcastle Upon Tyne based in modern single site premises. We have 6 GP partners who employ 2 salaried GPs, 2 Nurse Practitioners, 4 Practice Nurses, 3 Healthcare Assistants as well as a team of dedicated and supportive administrative staff. We currently have a patient list size of around 15,000. We are highly involved in our PCN (Inner West Newcastle) and through this we benefit from pharmacists, physiotherapists, psychology, paramedics, a nurse associate, mental health workers and social prescribers who also work for the practice. Our collective purpose is to provide the best care to our patients. We use SystmOne clinical system.
Job responsibilities
- Provide safe, evidence-based, cost-effective, and individualised nursing care needs as identified, or where care has been transferred by a GP. This will usually be in surgery but occasional home visits will also be required.
- Have responsibility for individual patient caseload, ensuring all patients have an accurate plan of care, which reflects the assessment undertaken and incorporates the issues and recommendations made.
- Initiating and maintaining health screening and promotion clinics for identified areas of the practice population, recognising where modification of systems or referral is necessary.
- Managing the nursing care of patients with chronic disease, employing personal recall systems suitable to the practice needs.
- Undertake general nursing examinations and treatments for patients' healthcare needs such as wound care, sutures removal, electrocardiogram (ECGs), Cytology, Ear Care.
- Maintaining efficient liaison with all areas of the Primary Care Team and other agencies to assure continuity of care where necessary.
Health Promotion
- Undertake basic assessment of the needs of patients including men and women's health screening and education.
- Coordinate the development of plans to meet the health needs of patients including diagnostic tests, treatment plans, health promotion and prevention.
- Provide smears following current guidelines and manage follow-up system.
- Administer depoprovera injections following current guidelines.
- Be able to assess critically and re-evaluate the clinical situation as the patient's condition or needs change.
- Assist in clinical audit and the setting up and monitoring of standards of care.
- Assist and teach others in clinical setting, including unqualified staff.
- Work effectively in the team.
Information and IT
- Facilitate good communication systems within the practice with the use of computerised messages.
- Extrapolate information received by outside agencies and bring to the notice of all clinicians wherever necessary.
- Undertake all training and attend any computer data input and retrieval meetings deemed necessary by the Practice Manager to manage the clinical system effectively.
Personal Development
- Attend relevant training updates as required to maintain clinical effectiveness.
- Maintain continuous professional development records in keeping with the requirement of NMC.
Person Specification
