Job Description

  • Providing expert services and support to patients registered with our Hub
  • Offering clear and accessible advice on medicines, repeat prescriptions, and general pharmaceutical queries
  • Assisting with prescribing, medicines optimization, and high-risk drug management audits across primary care sites
  • Using critical thinking and effective communication skills to assess and address patient needs
  • Prescribing urgent prescriptions and ensuring accurate re-prescribing using summary care record information
  • Identifying and addressing clinical risks in line with professional responsibilities
  • Keeping clear, accurate, and timely records of all consultations and advice given
  • Actively engaging with colleagues to deliver outstanding service
