We are seeking a motivated and experienced Clinical Pharmacist to join our team and provide high-quality clinical support across two well-established GP practices in Camden. This is an excellent opportunity for an enthusiastic pharmacist looking to play an integral role within primary care and deliver meaningful patient-focused services.

Key Responsibilities

  • Provide clinical support across both GP practices, ensuring safe and effective prescribing
  • Conduct medication reviews, structured medication reviews (SMRs), and support long-term condition management
  • Work closely with GPs, nurses, and the wider MDT to optimise medicines use and improve patient outcomes
  • Assist with audits, QOF-related work, and clinical projects within the PCN
  • Support the development and delivery of repeat prescribing processes and medicines optimisation initiatives

Essential Criteria

  • Independent Prescriber (IP) qualification
  • CPP or equivalent experience
  • Proven experience working within a PCN or GP practice environment
  • Strong clinical judgement and the ability to work autonomously as well as part of a multidisciplinary team
  • Excellent communication and organisational skills
