Key Duties:

  • Perform medication reviews and optimisation by conducting comprehensive medication reviews for patients to ensure safe, effective, and appropriate use of medications.
  • Prescribe and re-prescribe medications in accordance with clinical guidelines, patient needs, and organisational long-term conditions by supporting patients with chronic diseases (diabetes, hypertension etc) by reviewing and optimising treatment plans.
  • Liaising with the MDT, communicate with hospital pharmacists or specialists to discuss complex cases or medications that require specialised support.
  • The successful applicant must be comfortable to mentor trainee support staff and pharmacists.

  • Salary IRO £43,000 - £54,000 pro-rata
