Be the Mental Health Leader Patients Need - Right at the Heart of Primary Care. We are recruiting a part-time (30 hours) Enhanced Practice Mental Health Nurse to join our established, experienced, and forward-thinking team. Are you passionate about transforming mental health support where people live, work, and thrive? We are looking for a dedicated Registered Mental Health Nurse to join our innovative, compassionate, and community-focused team. In this role, you will support people with mild to moderate mental health needs right at the heart of primary care where early support can have the greatest impact.
You will work alongside GPs, Physiotherapists, Clinical Pharmacists, Occupational Therapists, Podiatrists, Social Prescribers, and other allied professionals to offer joined-up care that puts patients first. Together, we are rethinking how mental health is managed in General Practice and we want you to be part of it.
Main duties of the job
As a Mental Health Nurse working at Enhanced Level Practice, you will play a central role in delivering the PCN's mental health offer. You will be embedded within practices, delivering early support to adults with low-to-moderate mental health needs, while maintaining strong relationships with local Community Mental Health Teams and VCSE partners. You will help reduce referrals to crisis or specialist services by offering proactive, preventative, and person-centred care. You will be expected to work at enhanced level: managing complexity, leading service development, and actively shaping a population-based mental health offer.
Job responsibilities
- Deliver timely, person-centred care for adults with low to moderate mental health needs referred from within the PCN, whether through GP triage, direct referral, or proactive outreach.
- Conduct comprehensive mental health assessments and develop shared care plans with patients.
- Deliver brief interventions, psychoeducation, and proactive medication support (where appropriately trained).
- Refer and signpost to IAPT, CMHTs, crisis services, and VCSE organisations as required.
- Work collaboratively with GPs, ANPs, Social Prescribers, OTs, and other team members to plan joined-up care.
- Work autonomously and confidently to manage complexity and psychosocial need.
- Apply holistic clinical reasoning and shared decision making to develop meaningful care plans.
- Apply safeguarding principles and respond promptly when crises arise.
- Actively contribute to mental health MDTs and population health discussions at PCN level.
Evidence, Research, and Development
- Contribute to service development, quality improvement, and alignment with mental health strategy.
- Promote public mental health, prevention, and early intervention as part of population health goals.
- Identify high-risk populations, promote mental wellbeing, and address social determinants of mental health (e.g., loneliness, housing, debt).
- Act as a clinical role model within the MDT, supporting professional development and reflective practice.
- Record activity and outcomes in line with reporting requirements.

Experience, Knowledge and Qualifications
- Registered Mental Health Nurse with the NMC.
- Proven experience supporting adults with mental health needs, ideally in a community or primary care setting.
- Strong understanding of common mental health conditions and evidence-based interventions.
- Excellent communication, triage, and consultation skills.
- Ability to work independently while contributing effectively to a wider MDT.
- Up to date CPD portfolio demonstrating knowledge of the Promni Health and General Practice Nursing Career and Core Capabilities Framework and the ability to operate at enhanced level practice.
- Experience working within or alongside a GP practice (desirable).
