Be the Occupational Therapist Patients Need - Driving Proactive Frailty Care in Primary Care.

Are you passionate about delivering proactive, preventative care that keeps people well and independent in their communities? We are looking for a dedicated Occupational Therapist to join our forward-thinking, multidisciplinary primary care team on a fixed term contract until 31/03/2027.

In this role, you'll deliver home-based functional assessment and intervention, focused on independence, home safety, and preventing deterioration in people living with frailty. As the role includes home visits, you'll need to be a car driver with access to a vehicle.

The successful candidate will deliver home-based assessment and intervention for patients living with frailty, focusing on functional independence, activities of daily living (ADLs), and environmental risk management. The role centres on improving a patient's ability to safely manage at home, through targeted intervention addressing functional decline, cognitive challenges, and environmental barriers.

Working as part of the Primary Care Network multidisciplinary team, you will support proactive frailty management, contributing to shared care planning and coordinated, person-centred care across primary, community, and social care services.
