Grimsby Full-Time On-site

Two Registered Mental Health Nurses are sought for the prevention team in a primary care network, based at a community hub whose exact site is still to be confirmed. The work centres on assessment, risk management and brief intervention for adults with mild to moderate mental health needs, with onward referral where needed. • Carry out comprehensive mental health assessments, formulation and shared care plans
  • Manage a clinic template of around 80 slots a week, roughly 13-14 patient contacts a day
  • Support patients with depression, anxiety, sleep issues and adjustment disorders
  • Deliver brief interventions, psychoeducation and proactive medication support where trained
  • Refer and signpost to Talking Therapies, CMHTs, crisis services and VCSE organisations
  • Apply safeguarding principles and act quickly when crises arise
  • Work collaboratively within the MDT to plan joined-up care
  • Complete mental health care plans to support QOF indicators
  • Contribute to service development, quality improvement and PCN mental health discussions
  • Identify high-risk populations and tackle social determinants of mental health
  • Record activity and outcomes in line with reporting requirements
