Purpose of the Job
To work closely within a multi-agency team at Airedale General Hospital, provide one-to-one support in the community or people’s homes, and work with pregnant women and new mothers who need additional support.
Main Duties and Responsibilities
- Work in a multi-agency team supporting patients at Airedale General Hospital.
- Provide support to pregnant women and new mothers.
- Review and assess the needs of service users with an alcohol or drug problem within a variety of settings.
- Support service users to improve general health, including dietary needs and personal hygiene.
- Assist service users to improve their social situation, including housing, benefits, accessing community services and budgeting.
- Work with service users to remove structural and social barriers to accessing treatment services.
- Support individuals and families to access appropriate treatment and recovery services.
- Keep accurate and confidential records.
- Produce relevant documents and reports.
- Attend and contribute to meetings with other staff and partners.
Requirements
- Travel & Transport: The role requires frequent travel to multiple client sites, including locations not always served by public transport. Holding a full driving licence and having access to a vehicle would be beneficial.
- Compliance: Abide by the Organisation's Code of Conduct, health and safety and HR Policies.
Salary £21,282 (Actual), £26,603 (FTE) per year
Hours Part-time, 28 hours per week
Location Keighley and Bradford
