We are seeking a dedicated and highly skilled Clinical Pharmacist to join a dynamic healthcare team in Liverpool . In this role, you will work closely with doctors, nurses, and other healthcare professionals to provide exceptional pharmaceutical care to their patients. You will be instrumental in ensuring the safe and effective use of medications, supporting patient care, and contributing to the overall success of our clinical services. Key Responsibilities: Review and monitor patient prescriptions to ensure appropriate medication therapy. Provide expert advice on drug interactions, side effects, and optimal drug therapies. Participate in multidisciplinary team meetings to support patient care planning. Conduct medication reviews for patients, particularly those with complex needs. Ensure compliance with clinical guidelines and best practices for medication safety. Provide education and training to staff and patients about medication use. Promote safe, effective, and cost-efficient use of medicines within the healthcare system. Contribute to audits, clinical trials, and research projects as required. Support the development and implementation of policies and procedures relating to pharmacological care.
