Are you a passionate, clinically driven Pharmacist looking to make a real impact in community healthcare? An exciting opportunity has arisen for a Primary Care Network (PCN) Pharmacist to join a dynamic and supportive team across Cornwall.  Key Responsibilities: Conduct structured medication reviews and support medicines optimisation Manage repeat prescribing systems and ensure safe, effective prescribing Provide clinical advice to patients and the practice team Support chronic disease management and preventative care initiatives Contribute to audits, QOF, and IIF targets within the PCN Liaise with community pharmacy teams, secondary care, and wider services About You: GPhC-registered Pharmacist Experience in a primary care, community, or hospital setting Independent Prescriber (desirable, or willingness to undertake training) If you would like to hear more about this role, please get in touch with Ibraim and he will be happy to discuss this further
