Full clean Driving Licence essential for this role.
We provide support on a 24/7 basis with a staffing ratio of 2:1 support. You will be expected to support on a rotational basis working 12-hour shifts. This will include every other weekend and will include sleep-in duties.
For this position it is essential that you have a background in supporting people with Psychosis, Bipolar disorder, and Autism. Ideally your experience will be in community-based settings, such as supported living and/or residential care. Knowledge of Positive Behaviour Support (PBS) would be an advantage.
You will play a pivotal role in providing person-centred support for a sweet natured, captivating and humorous gentleman. He is supported on a 2:1 basis in his own home. The person has a love for crazy golf, amusement arcades, board games, strolls on the beach, playing pool and visiting the pub. You should be fun, upbeat and have a naturally bubbly personality. You will also need to be calm, relaxed and resilient.

As a key team player your responsibilities will include:
- Providing person-centred support.
- Working with the person to help identify potential triggers which may lead to signs of mental health decline and behaviours of concern.
- Implementing Positive Behaviour Support (PBS) strategies to improve quality of life and reduce behaviours of concern.
- Collaborating with the person to create and implement tailored support plans.
- Assisting the person to develop essential life skills such as cooking, personal hygiene, budgeting, and household management.
- Supporting with medication, medical appointments, and maintaining good quality health and well-being.
- Supporting with community activities and enabling the person to engage in social events, access local services, and build meaningful relationships.
- Maintaining accurate and comprehensive care records, documenting progress, incidents, and outcomes as required.
- Working closely with your colleagues, the family, external professionals, and other agencies to ensure holistic care for the person.
