Community Emergency Care/Nurse Practitioner in Oldbury

Main area Emergency Care Practitioner/Emergency Nurse Practitioner (Urgent Community Response) Grade NHS AfC: Band 7 Contract Permanent. You will be required to work between the hours of 08.00hrs and 20.00hrs, 7 days per week as per the agreed roster. Hours Full time - 37.5 hours per week (Will cover a 7 day service 08:00am -20:00pm).

An exciting opportunity has arisen for an experienced Emergency Practitioner with a background in Nursing or Paramedicine to join the Urgent Community Response Services at Sandwell and West Birmingham NHS Trust. The post holder will be a registered Nurse, Paramedic, or Allied Health Professional who, acting within their professional boundaries, will deliver comprehensive care to referred patients. This will include conducting initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. They will demonstrate safe and effective clinical decision‑making and provide expert care for patients within the Urgent Community Response Service.

The post holder will work collaboratively with the multi‑disciplinary Community and Primary Care Network Team to address the needs of patients effectively and holistically. On successfully completing a 12 month period and trust ePortfolio, mapped to the Virtual Ward and Urgent Community Response Capabilities Framework, the successful candidate will progress to a Trainee Advanced Clinical Practitioner role, as part of their ECP/ENP role over 3 years, at the next available cohort of MSc at the approved Higher Education Institution, through an apprenticeship/NHSE funded route. The post holder will primarily be based at Sandwell Council House.

Main duties of the job:

• The post holder will be a registered Nurse, Paramedic, or Allied Health Professional who, acting within their professional boundaries, will deliver a high standard of patient care as an Emergency Practitioner within the Urgent Community Response (UCR) Service.
• Using enhanced autonomous clinical skills and a broad, in-depth theoretical knowledge base, the post holder will assess and triage patients, including providing a two‑hour response when appropriate.
• Deliver definitive treatment, including prescribing medications in line with policies, Patient Group Directives (PGDs), NICE guidelines, local clinical protocols, and care pathways.
• Make referrals to appropriate members of the primary, community, or acute care teams when necessary.
• Critically evaluate and analyse complex clinical problems, applying expertise and knowledge to inform decision‑making.
• Consult with colleagues to devise and evaluate complex care plans tailored to patients’ specific needs within their field of expertise.

This role requires a proactive approach to patient care, with the ability to work collaboratively across multidisciplinary teams while maintaining the highest clinical standards.

Person specification:

• Educated to a degree level in Nursing.
• Has completed consolidation of learning period as a newly qualified Nurse.
• Nursing & Midwifery Council (NMC) registration.
• Has completed Emergency Practitioner education programme at a recognised Higher‑Level Institution.
• Educated to a degree level in Paramedicine or equivalent experience.
• Has completed consolidation of learning period as a newly qualified paramedic or AHP.
• Health & Care Professions Council (HCPC) registration.
• Full UK driving licence.
• For purpose of MSc Advanced Practice Apprenticeship funding need GCSE English and Mathematics at grade C or above (or equivalent qualifications) at start of apprenticeship.
• Minor Injuries and / or Minor Illness Course.
• Extensive experience within either an ECP or ENP role working across both minor illness and injury.

Experience:

• Extensive post‑qualification experience as a Band 6 (or equivalent) Nurse.
• Extensive post‑qualification experience as a Band 6 (or equivalent) paramedic/working in an acute or community hospital setting as an ENP / ECP.
• Completion of Minor injuries/ illness Assessment course with portfolio evidence to demonstrate capability.
• Management of minor injury & minor illness.
• Fracture management.
• Recent and continued evidence of developing relevant practice.
• Has an in‑depth understanding of local and national issues relating to urgent care.
• Excellent knowledge base in the management of minor illness / injury.
• Experience of multidisciplinary team working.
• Able to demonstrate effective teaching and development of staff and students.
• Knowledge of Clinical Governance agenda.
• Knowledge of principles of Infection Control.
• Knowledge of Health & Safety at Work.
• Able to promote equality, diversity and rights.
• Experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans.
