District Nurse in Guildford

You will work collaboratively with the multi-disciplinary team to visit patients in their own homes and care homes to avoid unnecessary hospital admissions.

We have lots of training and development pathways at every level from band 3 to band 8. We provide a preceptorship programme to support you in your role and ongoing training and development. We use electronic clinical records through EMIS, and a work iPad and iPhone will be provided. This role has been identified as eligible for the joining incentive of a £2,000.00 phased payment. You will receive a joining incentive payment of £500 after 3 months’ service, a further joining incentive payment of £500 after 6 months’ service, with a final payment of £1,000 after a full 12 months’ service with Procare Community Services.

We are seeking a highly motivated and ambitious District Nurse to join the wider Community Nursing Team which includes Community Matrons, District Nurses, Registered Nurses, Nursing Associates, and Healthcare Assistants. You will undertake clinical skills including venepuncture, wound care, catheterisation, and end of life care including syringe drivers. You will manage a caseload of patients attached to 1 or 2 GP surgeries. The role will include caseload management of patients with a range of long-term conditions and multiple pathologies. You will provide holistic assessment of health needs relevant to nursing care and offer health promotion. You will need to have a District Nursing Qualification and experience of managing a team. You will act as a role model, teaching and supporting staff and students in the community.

Post holders will be available to work Monday to Sunday between 8am and 8pm. A full UK driving licence and vehicle for business use is required for this post; however, reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010. Detailed job description and main responsibilities:

• Maintains strong working relationships and communicates effectively with GPs, practice nurses, Social Services, Mental Health, acute/community hospital colleagues, Borough Council staff, and the local voluntary sector - particularly for patients receiving shared care to ensure an effective flow of patient information to ensure high-quality care.
• Attends all Gold Standard Framework and MDT meetings.
• Ensures regular meetings are in place with GP and Practice Nurse colleagues.
