Procare Health Ltd is seeking a Band 6 Senior Nurse for Urgent Community Response to join the Adult Community Nursing Service. You will deliver urgent, unplanned and planned care to adults in their own homes, helping prevent hospital admissions and promote independence.
You will use advanced clinical skills to assess, plan and deliver evidence-based care for patients with complex health needs, working with a multidisciplinary team and supporting the two-hour UCR standard.
