You will be part of a friendly and supportive local community nursing team, working collaboratively with colleagues across acute hospitals, social care, mental health services, GP practices, and Practice Nurses to provide seamless, person-centred care. To support you in your role, you will be equipped with an iPad and iPhone and will use the EMIS electronic patient record system, enabling you to work efficiently and effectively in the community.

Main duties of the job

Are you passionate for high quality patient care and experience? We are seeking a highly motivated Registered Nurse to work within a wider team of nurses and health care assistants. You will work under the direction of a District Nurse, to assess, implement and evaluate person centred care for a caseload of patients, undertaking venepuncture, wound care, catheterisation, syringe drivers, end of life care and symptom control. On achieving all relevant competencies, the post holder will be reviewed for a Band 6 Senior Community Nurse post. Post holders will be available to work Monday to Sunday between 8am and 8pm.

A full UK driving license and vehicle for business use is required for this post, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010. Procare Community Services are not a UKVI sponsoring organisation, so all candidates are required to have their own independent right to work.

Detailed job description and main responsibilities

The post holder will visit adult patients in their own home who have an identified nursing need including those with a long-term condition, complex care needs and needing care at home and the end of life. The post holder will use an electronic patient held record system, EMIS as a record of care and all our nurses are provided with a work iPhone and iPad for mobile working. The community nurse will use a range of nursing skills including venepuncture, wound care, catheterisation, syringe drivers, end of life care and symptom control and on successfully attaining all the competencies required will be reviewed for a Band 6 Senior Community Nurse post.

Person specification

  • Registered Level 1 Nurse with current Nursing and Midwifery Council registration
  • Supervisor / Mentorship qualification (within the first 2 years of post)
  • Evidence of continuing professional development
  • Ability to assess nursing needs and implement nursing care and treatment
  • Ability to prioritise visits in accordance with patient clinical need
  • Ability to organise own workload without direct supervision
