This unique position offers you the opportunity to work as part of a highly skilled community workforce who are the cornerstone of safe patient care. You will work collaboratively with the multi-disciplinary team to visit patients in their own homes and care homes to avoid unnecessary hospital admissions. We have lots of training and development pathways at every level through from band 3 to band 8. We provide a preceptorship program to support you in your role and ongoing training and development. We use electronic clinical records through EMIS and a work iPad and iPhone will be provided.
Main duties of the job include:
- Seeking a highly motivated and ambitious Senior Registered Nurse to join the wider Community Nursing Team.
- Working under the direction of a District Nurse to assess, implement and evaluate person centred care for a caseload of patients.
- Undertaking clinical skills including venepuncture, wound care, catheterisation and end of life care.
- Available to work Monday to Sunday between 8am and 8pm.
- A full UK driving licence and vehicle for business use is required for this post, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
Communication responsibilities include:
- Communicating effectively with patients, carers, and all members of the multidisciplinary team.
- Maintaining accurate and legible documentation and clinical records in line with professional and organisational requirements.
- Acting as the nominated Lead Nurse and overseeing daily handover.
- Maintaining strong working relationships with GPs, practice nurses, social services, and other healthcare professionals.
- Attending GP/case load meetings and MDT meetings if the District Nurse case load holder is unable to.
Service Development involves:
- Actively participating in the development and implementation of adult community nursing service developments and initiatives.
- Promoting and supporting the implementation of innovation and technology within the workplace.
- Participating in clinical audit and implementing changes to ways of working based on findings.
Leadership and Management tasks include:
- Supporting the District Nurse with operationally managing Registered Nurses in the community nursing team.
- Monitoring stock levels of dressings and other equipment.
- Ensuring that all complaints, incidents, and areas of clinical risk are reported accordingly.
Accountability for self and others includes:
- Recognising own limitations in the provision of clinical care and referring to other health care professionals as necessary.
- Keeping up to date with professional nursing and regulatory requirements.
- Participating in own annual appraisal and objective setting.
Qualifications required:
- Registered Level 1 Nurse with current Nursing and Midwifery Council registration.
- Educated to diploma or level 6/degree in relevant area.
Skills, Knowledge & Experience:
- Experience undertaking holistic assessments of patients with complex health and social needs.
- A broad range of clinical skills relevant to Community Nursing.
- A full UK driving licence and vehicle for business use is required.
