Procare Community Services is recruiting a Band 6 Senior Community Nurse for the Night Nursing Service covering Guildford and Waverley. You will deliver autonomous nursing care in patients’ homes during out-of-hours, supporting those with complex conditions and end-of-life care.
You will triage referrals, work with GPs, hospices and multidisciplinary teams to prevent hospital admissions, and mentor junior staff while contributing to service improvements.
