Procare Community Services Night Nursing Service seeks a Band 6 Senior Community Nurse to provide expert care to patients in their own homes out of hours.
You will assess, plan, deliver and evaluate nursing care for those with complex needs, triage referrals autonomously and coordinate with GPs, hospices and hospital teams, while keeping EMIS records up to date. A full UK driving licence is required with a vehicle for business use where needed.
