Priory Medical Centre is excited to welcome an enthusiastic and skilled Advanced Clinical Practitioner (ACP) to our progressive, patient-focused team in the heart of Warwick. In this role, you will lead on the management of Long Term Conditions, with a particular focus on Diabetes, playing a crucial part in improving health outcomes for our diverse patient population. Alongside this, you will run same-day acute triage clinics, using your advanced skills to assess, diagnose, treat, and prescribe for a wide range of presentations. You will work autonomously while being fully supported by our multidisciplinary team of GPs, nurses, paramedics, pharmacists, and health & wellbeing staff.

Main duties of the job
- Lead on the management of Long Term Conditions, with a focus on Diabetes, delivering comprehensive reviews, personalised care plans, medication optimisation, and proactive education.
- Run same-day acute triage clinics, assessing, diagnosing, treating, prescribing, and referring for a wide range of presentations.
- Handle undifferentiated conditions with confidence, making safe, timely decisions.
- Contribute to safeguarding, quality improvement, chronic disease management pathways, and collaborative MDT working.
- Support continuity of care and improved quality of life across our population.
About us
Priory Medical Centre is a well-established Warwick GP Partnership with a list size of approximately 18,900 patients and one branch surgery. We have 5 GP Partners and a full complement of additional staff including Salaried GPs, Advanced Nurse Practitioners, Practice Nurses, Health Care Assistants and administrative support. We are a recognised training practice for GP Registrars.
In June 2021, the practice moved into a brand new purpose-built surgery designed with patient care in mind. The new building will be a modern, energy efficient & fit for purpose building that will enable the practice to provide better services to our patients.
Job responsibilities
- Practice in compliance with their respective code of professional conduct and within their scope of practice.
- Demonstrate a critical understanding of their broadened level of responsibility and autonomy.
- Act on professional judgement about seeking help, demonstrating critical reflection on own practice.
- Develop, implement and embed health promotion and wellbeing programmes.
- Manage patients presenting with a range of acute and chronic medical conditions.
- Implement and evaluate individual specialised treatment plans for chronic disease patients.
- Identify, manage and support patients at risk of developing long-term conditions.
- Demonstrate effective communication skills, supporting people in making decisions.
- Initiate, evaluate and modify a range of interventions which may include prescribing medicines.
- Exercise professional judgement to manage risk.
- Maintain accurate clinical records in conjunction with extant legislation.
- Support the team in dealing with clinical emergencies.
- Recognise, assess and refer patients presenting with mental health needs.
- Support in the provision of vaccination programmes for both adults and children.
- Contribute to practice targets (QOF, etc.).
- Liaise with external services/agencies to ensure the patient is supported appropriately.
- Deliver opportunistic health promotion where appropriate.
Person Specification
- Qualified healthcare professional educated to Masters degree.
- Evidence of working at an enhanced level.
- Registration with their professional governing body.
- Experience of practice within the four pillars.
- Good interpersonal and organisational skills.
- Ability to follow legal, ethical and professional policies.
