As a Mental Health & Addictions Triage Practitioner, you will review and triage referrals, assessing individuals' needs and directing them to the most appropriate treatment pathway.

You will provide guidance and signposting where alternative support is more suitable, while managing patient enquiries with empathy and professionalism., As a Mental Health & Addictions Triage Practitioner, you will review and triage referrals, assessing individuals' needs and directing them to the most appropriate treatment pathway.

You will provide guidance and signposting where alternative support is more suitable, while managing patient enquiries with empathy and professionalism.

You will assess and escape risk and safeguarding concerns, maintain accurate clinical records, and work closely with referral coordinators and clinical teams to ensure a seamless patient journey.

Acting as a clinical point of contact within the Customer Service Centre, you will support colleagues, contribute to safe and effective case management, and participate in supervision and ongoing professional development.
