- Lead on patients physical healthcare assessments, care planning and ongoing reviews.
- Provide specialist advice on areas such as diabetes, hypertension, COPD and wound care.
- Lead Infection Prevention & Control across the site, including audits and outbreak management.
- Deliver physical healthcare interventions including ECGs, phlebotomy and wound care.
- Liaise with primary care services to coordinate patients ongoing healthcare needs.
- Support and train nursing staff in physical healthcare, observations and clinical assessments.
- Lead health promotion and smoking cessation initiatives.
