Main duties of the job
- You will be part of a compassionate team who are committed to our patients' wellbeing.
- We take a personalised approach, using proven therapies to help people move forward with confidence and build a healthier, more fulfilling future.
- You will play a key part in their recovery, assisting qualified staff with the assessment and implementation of individual patient care plans, undertaking routine tasks and activities as directed or delegated to facilitate wellbeing, dignity and treatment of patients.
- Receives and responds promptly to referrals, undertaking consultations and assessments to determine effective dietary and nutritional plans for assigned clients using appropriate interventions.
- Plans, monitors and reviews dietary and nutritional interventions ensuring they continue to meet the needs of the patient and are consistent with the provisions detailed in the clients' general therapy plan.
- Manages, monitors and reviews an assigned caseload, providing clinical supervision of dietary and nutritional plans and through regular consultation and liaison with professional and clinical colleagues.
- Maintains accurate and up-to-date records in the form of case notes, case files, clinical reports, computerised system information and other associated documentation, presenting and sharing relevant information and knowledge concerning assigned clients with professional and clinical colleagues at regular multi-disciplinary team meetings.

Person Specification Requirements
- The ideal candidate will hold a degree in dietetics or nutrition, along with a relevant post-graduate qualification in a specialized area.
- They must be HPC registered and a member of a recognised professional institution.
