We are looking to recruit a support worker to join our team in Wolverhampton. This is a full‑time role, 07:00 – 22:00 (1 sleep‑in per week 22:00 – 07:00). £13.45 per hour.
Responsibilities
- You will be part of a compassionate team committed to our residents' wellbeing, using proven therapies to help people build a healthier future.
- Supporting patients with assessment and implementation of individual care plans and routine tasks.
- Providing day‑to‑day activities support, encouraging and promoting independent living.
- Assisting with personal care, participation in activities and daily living skills including tenancy maintenance.
- Completing daily electronic and paper record administration with the individual's involvement as appropriate.
- Liaising with specialists to maintain residents' health and wellbeing.
- Facilitating community participation and integration through activities that meet residents' ambitions.
- Embedding the REACH standards in everyday practice.
- Following company policies and procedures.

Disclosure and Clearance
