Job Title: Community Link Worker – Social Prescribing\\nLocation: Derby (Derby City South PCN)\\n Salary: £25,750 per annum\\n Contract Type: Full-Time, Permanent (subject to continued funding)\\n Hours: 37 hours per week\\n Working Pattern: Monday to Friday, with occasional evening or weekend work (time off in lieu)\\n Start Date: ASAP\\n Driving Requirement: Full UK Driving Licence & Access to Own Vehicle (Essential)\\n\\nAbout the Role\\nWe are recruiting a compassionate and motivated Community Link Worker to support individuals referred via GP practices as part of a Social Prescribing service within Derby City South Primary Care Network.\\nThis role focuses on supporting people who may be experiencing social, emotional or practical challenges that impact their health and wellbeing, such as loneliness, mental health difficulties, housing issues, financial hardship or long-term health conditions.\\nUsing a person-centred and trauma-informed approach, you will work alongside individuals to understand what matters to them, co-produce personalised wellbeing plans and connect them to appropriate community, voluntary and statutory services. The aim is to empower individuals to improve their independence, resilience and overall quality of life.\\n\\nKey Responsibilities\\n\\nReceive and manage referrals from GP practices\\nBuild trusting, person-centred relationships with individuals\\nTake a holistic approach to understanding needs, strengths and goals\\nCo-produce personalised wellbeing action plans\\nSupport individuals to access community groups, services and activities\\nEncourage independence, self-management and social connection\\nUse coaching and motivational approaches to support behaviour change\\nIdentify and manage risk appropriately, escalating concerns where required\\nMaintain accurate records using clinical and organisational systems\\nWork collaboratively with GP practices, NHS partners and community organisationsPartnership & Community Engagement\\n\\nBuild effective working relationships with GP practices and PCN staff\\nMaintain up-to-date knowledge of local community services in Derby\\nPromote social prescribing and its benefits across health and community settings\\nSupport the development of strong referral pathways between servicesWhat We’re Looking For\\nEssential:\\n\\nExperience supporting individuals in a health, care, community or voluntary setting\\nStrong person-centred and strengths-based approach\\nAbility to build rapport and work in a non-judgemental way\\nExperience managing or coordinating support for individuals\\nGood understanding of social issues affecting health and wellbeing\\nConfident working independently and managing a caseload\\nStrong communication and IT skills\\nFull UK driving licence and access to a vehicle\\nEnhanced DBS (or willingness to obtain)Desirable:\\n\\nExperience within the voluntary and community sector\\nKnowledge of local services in Derby\\nTraining in motivational interviewing or coachingWhat’s in It for You\\n\\n£25,750 annual salary\\nMeaningful, community-focused role making a real difference\\nSupportive, values-led working environment\\nFull induction, training and ongoing development\\nOpportunity to shape and grow social prescribing provision locallyValues\\nInnovative | Dynamic | Dependable | Respectful | Kind\\nThese values underpin the approach taken to support individuals and communities.\\nApply Now\\nIf you are passionate about empowering people, reducing health inequalities and working within a community-based, person-centred service, we would love to hear from you
