In this role, you will provide vital nursing services directly in patients' homes, helping them manage conditions, recover safely, and maintain independence.
Role Overview
As a Registered Nurse in home care, you will be responsible for providing high‐quality patient care, collaborating with physicians and multidisciplinary team members, and ensuring the smooth operation of the healthcare facility.
Responsibilities
- Perform comprehensive in‐home nursing assessments and re‐assessments
- Develop, implement, and revise individualized care plans in accordance with physician orders
- Provide skilled nursing services including wound care, injections, catheter care, and medication administration
- Monitor patient status and response to treatments, documenting all care and changes in condition
- Educate patients and caregivers on disease management, medication use, and safety practices
- Coordinate with other healthcare professionals including PTs, OTs, HHAs, and social workers
- Ensure compliance with all Article 36 regulations and agency policies
- Participate in quality assurance activities, case conferences, and staff meetings
The Ideal Candidate
- Graduate of an accredited nursing program (Associate or Bachelor's degree)
- Minimum of 1 year of experience in home health, hospital, or clinical setting preferred
- Strong clinical judgment, communication, and patient education skills
- Self‐motivated and able to work independently in the field
- Must pass background checks, drug screening, and health clearances
- Valid driver's license and/or reliable transportation
- Bachelor's degree in Nursing (BSN)
