Join a dynamic team dedicated to providing exceptional home healthcare. We are seeking a licensed and motivated Occupational Therapist to join our home care team. As an OT, you will play a critical role in helping patients regain independence and improve their quality of life in the comfort of their own homes.
Role Overview: The Occupational Therapist will evaluate patients in their homes, develop personalized care plans, and implement therapeutic strategies to promote safety and functionality. Working alongside a multidisciplinary team, you'll help clients adapt to daily challenges and achieve their highest level of independence.
Responsibilities:
- Conduct in-home assessments of patients' physical, cognitive, and functional abilities
- Develop individualized treatment plans focused on improving daily living and motor skills
- Provide direct therapeutic interventions and adaptive equipment training
- Educate patients and caregivers on strategies to promote safety and independence
- Recommend home modifications and equipment as needed
- Collaborate with nurses, physical therapists, social workers, and aides for coordinated care
- Participate in care planning and team meetings as required
The Ideal Candidate:
- Current New York State Occupational Therapist license
- Graduate of an accredited Occupational Therapy program
- Minimum of 1 year experience in home health, rehab, or clinical setting preferred
- Strong assessment, clinical, and communication skills
- Ability to work independently and manage time effectively
- Reliable transportation and willingness to travel to patient homes
- CPR certification and up-to-date health requirements
