In this role, you’ll provide vital nursing services directly in patients’ homes, helping them manage conditions, recover safely, and maintain independence.
Role Overview
As a Registered Nurse in home care, you will be responsible for providing high-quality patient care, collaborating with physicians and multidisciplinary team members, and ensuring the smooth operation of the healthcare facility.
Responsibilities
- Perform comprehensive in-home nursing assessments and re-assessments
- Develop, implement, and revise individualized care plans in accordance with physician orders
- Provide skilled nursing services including wound care, injections, catheter care, and medication administration
- Monitor patient status and response to treatments, documenting all care and changes in condition
- Educate patients and caregivers on disease management, medication use, and safety practices
- Coordinate with other healthcare professionals including PTs, OTs, HHAs, and social workers
- Ensure compliance with all Article 36 regulations and agency policies
- Participate in quality assurance activities, case conferences, and staff meetings
The Ideal Candidate
- Graduate of an accredited nursing program (Associate or Bachelor’s degree)
- Minimum of 1 year of experience in home health, hospital, or clinical setting preferred
- Strong clinical judgment, communication, and patient education skills
- Self‑motivated and able to work independently in the field
- Must pass background checks, drug screening, and health clearances
- Valid driver’s license and or reliable transportation
- Bachelor’s degree in Nursing (BSN)
