About the role: We are looking for a caring and enthusiastic Senior Occupational Therapist to join our Hospice at Home team, supporting patients in the comfort of their own homes. You will take a holistic approach, supporting patients' physical, psychological, social and cognitive needs. Working both independently and within a multidisciplinary team, you will deliver personalised interventions that promote a rehabilitation and enablement-focused approach to care. You will build meaningful relationships with patients, families and carers, supporting them through challenging circumstances. Working alongside colleagues and external partners, you'll help ensure patients are supported to remain in their preferred place of care wherever possible.
