Job Summary
Join the Hospice at Home team as a Specialist Nurse/Practitioner, delivering compassionate care to patients in their homes. You will work closely with Clinical Nurse Specialists to support team development and help patients achieve their preferred place of care.
Responsibilities
- Provide high-quality palliative care to terminally ill patients in the community.
- Collaborate with the Clinical Nurse Specialists to develop and implement care strategies.
- Communicate effectively with patients, families, and multidisciplinary teams to understand and respond to care needs.
- Adapt to innovative ways of delivering care and support more patients at home.

- Relevant degree or diploma in nursing or healthcare.
