The Prince & Princess of Wales Hospice is a well-established charity located within Bellahouston Park, Glasgow. The hospice provides specialist palliative care for people with a life-limiting, progressive illness, and support for their families and carers.
The Prince & Princess of Wales Hospice is seeking a compassionate, motivated and forward-thinking Young Adult Service Lead to join our dedicated Young Adult Service. This is a unique opportunity to help shape and deliver specialist palliative care for young adults living with life-limiting conditions, ensuring they receive the highest standard of holistic, person-centred support.
About the role:
As the Young Adult Service Lead, you will play a pivotal role in the development, delivery and ongoing evolution of our Young Adult Service. Working closely with the Clinical Lead for the Young Adult Service, the Transition Lead and senior management, you will help drive the strategic direction of the service and ensure it continues to meet the needs of young adults and their families.
Key responsibilities:
- Collaborate with the Clinical Lead, Transition Lead and senior management to develop and implement the strategic vision for the Young Adult Service, ensuring alignment with hospice-wide objectives.
- Act as a core member of the multi-disciplinary team, taking responsibility for the assessment, planning, delivery and evaluation of care for young adults and their families.
- Provide holistic, specialist palliative care and clinical support to young adults with progressive illness and complex healthcare needs.
- Be a visible, approachable and responsive leader who embodies the hospice’s values and promotes a culture of compassion, dignity and respect.
- Work with key partners across health boards and Health and Social Care Partnerships (HSCPs) throughout Scotland to map current transition pathways and support their development.
- Foster an environment that champions partnership-working with young adults and those important to them, ensuring their voices shape the care they receive.
- Promote excellence in specialist palliative care through education, audit, research and alignment with local, regional and national strategies.
