We are looking for an enthusiastic and highly skilled Advanced Nurse Practitioner (ANP) to join our friendly, forward thinking general practice team. This is an exciting opportunity to work autonomously within a supportive multidisciplinary environment, delivering high quality, patient centred care to our population. You will use your advanced clinical skills to assess, diagnose, treat, and manage patients presenting with a wide range of acute, chronic, and complex conditions. The role includes independent prescribing, clinical decision making, and contributing to service development within the practice.
Main duties of the job
- The post holder will be an experienced clinician acting within their professional boundaries.
- They will demonstrate safe, clinical decision-making and expert care for our practice population, from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care.
- They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
Job responsibilities
- Provide advanced clinical assessment, diagnosis, and management of patients.
- Manage acute presentations, long term conditions, and urgent on the day demand.
- Undertake clinical examinations, investigations, and interpret results to support clinical decisions.
- Prescribe medications independently in line with current legislation and clinical guidelines.
- Provide health promotion, disease prevention, and patient education.
- Work collaboratively with GPs, nurses, pharmacists, HCAs, and the wider PCN team.
- Contribute to QOF, enhanced services, and practice quality improvement initiatives.
- Maintain accurate, timely clinical records using EMIS.
- Participate in clinical meetings, audits, and governance activities.
Secondary Responsibilities
- Participate in local initiatives to enhance service delivery and patient care.
- Support and participate in shared learning within the organisation.
- Develop an area of specialist interest, taking the lead within the organisation.
- Continually review clinical practices, responding to national policies and initiatives where appropriate.
- Participate in the review of significant and near‑miss events applying a structured approach i.e., root cause analysis (RCA).
- Drive the development of nursing services within the organisation, liaising with external agencies and professional organisations as required.
- Develop practice administrative and clinical protocols in line with the needs of the patient and current legislation.
