We have an exciting opportunity for an experienced RGN to join our dynamic team in Rosewood Primary Care Network. Rosewood PCN includes the following GP Practices: Acorn Medical Practice, Churchside Medical Practice, Forest Medical Group, Millview Surgery, and Roundwood Surgery.

ABOUT THE ROLE

Employed through PICS but working for Rosewood PCN, we have an exciting opportunity for an RGN to join the PCN to support the practices in the delivery of Primary Care nursing to housebound patients. The role will be based in the community, delivering nursing care on behalf of the GP practices to patients in their own home including care homes. The team will consist of two RGNs and a qualified Nurse Associate and a Student Nurse Associate who are supported by a Care co-ordinator. There may be the requirement to occasionally deliver care within the GP practices and support PCN pilots and projects as required.

Main duties of the job:

  • Use clinical assessment skills to undertake annual reviews of housebound patients and develop management plans which may include: initiating / adjusting medications, referring to a wide range of health and social care services, promoting self-management.
  • Undertake frailty reviews, immunisation programmes and primary care clinical procedures.
  • Work closely with nursing and administrative colleagues in the team, leading and developing the service and team members.
  • Collaborate with GP practice colleagues to ensure seamless care between the community team and GP practice.
  • Refer patients to other health, social care and voluntary services as required.

KEY REQUIREMENTS

  • NMC registration as a Registered General Nurse.
  • Post-registration long term conditions education.
  • Primary or community nursing experience.

Job responsibilities:

  • Clinical Practice: To assess, design, implement and evaluate safe, creative and effective treatment plans in collaboration with the patients' diverse needs, their carers and other professionals.
  • Responsible for the safe monitoring, management and administration of medication as required by PICS policy.
  • To maintain safeguarding responsibilities for children and vulnerable adults in line with PICS policy, and Local Authority procedure and guidance, including making timely referrals as required.
  • To maintain accurate, timely and relevant clinical records both written and computerised.
  • To provide integrated care following a holistic assessment which enables patients to be cared for in their own homes and clinic settings.
  • The post holder will facilitate the prevention of unnecessary hospital admission/re-admission by reducing the effects of ill health and disability and maximising the individual's potential for independence.
  • To manage patients living with long-term conditions, including frailty, and provide relevant care as necessary.
  • This may include non-medical prescribing.

Immunisation:

  • Support the PCN GP Practices by carrying out immunisations within patient homes, including care homes.
  • Undertake opportunistic and domiciliary immunisations, following up on non-immunised patients; providing information, advice and updating dates as required.
  • Provide outreach contact to access care needs of patients as delegated by the practices.
  • Be responsible for carrying out a routine enhanced immunisation programme.
  • Collate, collect and monitor data to support immunisation statistical returns.
  • Work collaboratively with team members and partners to meet immunisation targets for the practice.

Communication:

  • Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
  • Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
  • Anticipate barriers to communication and take action to improve communication.
  • Act as an advocate when representing the patients and colleagues' viewpoints to others.

Delivery Quality Service:

  • Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC 2018).
  • Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
  • Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
  • Deliver care according to National Institute for Clinical Excellence (NICE) guidelines, local guidelines and evidence-based care.
  • Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.
  • Participate in the maintenance of quality governance systems and processes across the organisation and its activities.
  • Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required.

Team Working:

  • Understand own role and scope in the organisation and identify how this may develop over time.
  • Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working.
  • Delegate clearly and appropriately, adopting the principles of safe practice and assessment of competence of those taking on delegated duties.
  • Ensure clear understanding and utilisation of referral mechanisms within the practice.
  • Work effectively with others to clearly define values, direction and policies impacting upon care delivery.

Management of Risk:

  • Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients.
  • Monitor work areas to ensure they are safe and free from hazards and conform to health, safety and security legislation, policies, procedures and guidelines.
  • Ensure safe storage, rotation and disposal of vaccines and drugs is undertaken.
  • Apply infection control measures within the practice according to practice, local and national guidelines.

Learning and Development:

  • Disseminate learning and information gained to other team members in order to share good practice and inform others about current and future developments.
  • Assess own learning needs and undertake learning as appropriate.
  • Provide an educational role to patients, carers, families and colleagues in an environment that facilitates learning.
